Help Me Live Homecare provides:

-Personal care assistance

-Homemaker services

-Chore services

-Out-of-Home respite care

Services are provided by trained, background-checked caregivers under the supervision of our care coordinators.

## Scheduling and Availability

– Services are scheduled in advance based on availability

– We require 24-hour notice for schedule changes when possible

– Emergency situations will be accommodated as staffing allows

– Holiday and weekend services may have different rates

## Payment Terms

– Payment is due according to your service agreement

– We accept cash, check, credit cards, and insurance (where applicable)

– Late payment fees may apply after 30 days

– Returned check fee: $35

## Cancellation Policy

– **Client cancellations:** 4-hour minimum notice required to avoid charges

– **Weather/emergency cancellations:** No charge when we cannot safely provide service

– **Service termination:** Either party may terminate with 48-hour written notice

## Client Responsibilities

You agree to:

– Provide accurate health and contact information

– Maintain a safe environment for caregivers

– Be present or arrange access for scheduled visits

– Treat our staff with respect and courtesy

– Report any concerns promptly

## Our Responsibilities

We commit to:

– Provide qualified, trained caregivers

– Maintain appropriate insurance coverage

– Follow your care plan as agreed

– Respect your privacy and dignity

– Address concerns promptly and professionally

## Limitations

– We are not a medical provider and cannot provide skilled nursing care

– Emergency medical situations require calling 911

– We cannot provide care if the environment is unsafe for staff

– Services may be limited by weather or other safety concerns

## Privacy and Confidentiality

Your personal information is protected under our Privacy Policy and HIPAA regulations.

## Liability

Help Me Live Home Care carries appropriate liability insurance. Our liability is limited to the cost of services provided.

## Changes to Terms

We may update these terms as needed. Changes will be communicated in writing and posted on our website.

These terms are governed by the laws of Ohio.
